Outreach Counselor
Full-Time
Location: Holyoke, MA
Pay Rate: $18-20/hour

Position Description:
Responsibilities include working with several different individuals in their homes. Being able to acclimate to the different needs of each individual, services to that individual being based on most current ISP.

General Responsibilities:

  • Transportation to and from doctors’ appointments, being able to communicate with Doctors any concerns that may arise, being able to receive information from these doctors and relay it back to the Program Directors.
  • The ability to multitask on a daily basis.
  • Assistance in Activities of Daily Living (ADL) if necessary, or any other support needed to aid the individual in living independently.
  • Working knowledge of each individuals possible risks they may face in the community, being able to decipher when interference is necessary.
  • Medication administration, both MAP regulated and self-medicating with assistance.
  • Daily documentation on each individual, both progress notes and behavioral tracking.
  • Creative thinking for community involvement. with these individuals.
  • Maintains strict confidentiality of all organizational and client information and adheres to all applicable federal and state regulations in the performance of job duties.
  • Ability to maintain regular attendance.
Position Qualifications:
  • High school diploma or GED required.
  • Three years of experience in human services, outreach experience a plus.
  • CPR & 1st Aid certification required (or willingness to obtain prior to or upon hire).
  • Proactive Approaches to Behavioral Challenges (PABC) certification where required (or willingness to obtain prior to or upon hire).
  • MAP certification required (or willingness to obtain prior to or upon hire). 
  • Have and maintain a valid Driver’s License for a minimum of 6 months with an acceptable driving record. 
  • Reliable transportation/vehicle.
