About the Role
Working as part of our Certified Community Behavioral Health Clinic team, you’ll support clients both in the community and at our Greenfield and Northampton offices. You’ll coordinate services across behavioral health, medical, and social supports—removing barriers, building partnerships, and ensuring each person has what they need to move toward recovery and stability.
Key Responsibilities
Care Coordination & Navigation
- Provide life-changing care coordination for clients with persistent mental health and substance use challenges—in the community and in the office.
- Facilitate communication among mental health, substance use treatment, and medical providers to create a seamless, integrated care experience.
- Help clients access legal aid, housing, employment, education, benefits, substance use treatment, medical care, and other essential services.
Collaboration & Community Partnerships
- Work closely with your team to ensure each client receives timely, appropriate support.
- Build relationships with community providers and organizations to streamline access to services.
Documentation & Professional Growth
- Maintain timely, accurate documentation that supports continuity of care.
- Participate in trainings as needed; stay current on best practices.
- Use of electronic health records and documentation systems.
Minimum Requirements
- BA/BS in Social Work or a related field.
- 3+ years of experience in care coordination/case management.
- A compassionate, strengths-based communicator who is organized, persistent, and client-centered.
- Valid driver’s license and reliable vehicle for travel between community settings and our Greenfield/Northampton offices.
