Schedule: Monday-Friday, 9a-5p | Includes On-Call Rotation
Employment: Full-Time | Temporary Assignment with Opportunity for Permanent Placement (See Details Below)
Pay Rate: $21- $21.75/hr (Based on Experience)

At ServiceNet, we support individuals on their journey to healing and independence. Our Substance Use Services Division provides safe, structured, and supportive recovery homes for adults working toward sustained recovery. We’re looking for a committed Case Manager to join our team in a temporary capacity, with the opportunity to apply for a permanent position based on performance and organizational needs.
About the Role:
As a Case Manager, you’ll play a vital role in supporting residents as they navigate treatment, access resources, and build life skills that sustain recovery. Working in our Moltenbrey House, you’ll collaborate with staff and community providers to ensure individuals receive the care and support they need to succeed.
Key Responsibilities:
Client Support & Case Management
- Develop and maintain Individual Action Plans (IAPs) based on client goals
- Meet regularly with residents to review treatment progress and make referrals
- Monitor progress and support residents in navigating challenges to recovery
- Provide education on HIV/AIDS, tobacco use, Medication-Assisted Treatment (MAT), and access to healthcare
- Maintain and update Medication Administration Records (MAR)
Group Facilitation
- Lead a minimum of two psychoeducational, Health & Wellness, and Life Skills groups per week.
Documentation & Collaboration
- Complete enrollment and discharge summaries in Virtual Gateway
- Monitor and update client insurance information
- Submit clear, timely notes (case management, service, and log notes)
- Collaborate with community providers and attend community and provider meetings (e.g., OTF)
- Report progress and concerns to the House Manager and/or Clinical Supervisor
Team & Program Responsibilities
- Respond quickly to emergency situations and provide CPR/First Aid as needed
- Participate in regular supervision, team meetings, and required trainings
- Flex across ServiceNet’s Substance Use programs as needed
What You Bring to the Table:
- Experience or interest in substance use recovery and/or behavioral health
- Strong organizational and communication skills
- Confidence facilitating groups and managing multiple priorities
- Comfort working in dynamic, team-based environments
- Willingness to be part of an on-call rotation
- Ability to uphold confidentiality and adhere to HIPAA and 42 CFR Part 2 standards

- ServiceNet is seeking a candidate to fill a temporary assignment. At the conclusion of the temporary assignment, the candidate may be eligible to apply for a regular employment position based on their interest and qualifications.
