CCBHC Case Manager
Location: Greenfield, MA
Salary: $20.00 Per/hour

ServiceNet is seeking compassionate, dedicated, and enthusiastic individuals to join our team. As a Case Manager, you will be at the heart of transforming lives by helping clients facing persistent mental health and substance use challenges to live joyful and fulfilling lives. Key Responsibilities:
  • Provide life-changing care coordination for clients with persistent mental health and substance use challenges, both in the community and in the office.
  • Coordinate care with mental health and health care providers to create a seamless, integrated care experience.
  • Assist clients in navigating services such as legal aid, housing, employment, substance use treatment, medical care, and more, helping them overcome barriers to successful recovery.
  • Collaborate with your team to ensure each client receives the support they need to thrive.
  • Build partnerships within the community, making it easier for clients to access the resources they need.
  • Participate in trainings as needed
Qualifications:
  • A BA/BS in Social Work or a related field.
  • At least 3 years of experience in care coordination.
  • Someone who is compassionate, an excellent communicator, and ready to make a difference.
  • The ability to travel, with a valid driver’s license and reliable vehicle.
