TRIAGE COUNSELOR- PREP PROGRAM- HOLYOKE
$17/hr

We are seeking a full time counselor to work (M-F 9am-5pm) in our Prevention and Recovery Early Psychosis Program (PREP).

JOB OVERVIEW:
  • Support individuals, families, and health care providers in screening referrals and scheduling assessments.
  • Support individuals and families in completing registration paperwork and gather baseline and outcomes data as needed, including conducting the clinical interviews required by our DMH grant funding as needed
  • Work with participants to set up a welcoming environment by decorating the space, organizing and playing games (e.g., ping pong, basketball, chess and taking participants on outings (apple picking, bowling, trips to the beach)., to help young adult participants engage with PREP and with each other in order to foster socialization and recovery.
  • Provide individual school and work coaching to individuals as part of a multi-disciplinary treatment team
  • Provide case management support for individuals
  • Attend treatment team meetings and participate in weekly discussions about individuals
  • There will be opportunities to observe and participate in training and seminars at PREP and in community settings
  • Responsible for helping participants orient to PREP helping to identify and troubleshoot obstacles with the participant’s clinical team.

POSITION QUALIFICATIONS:
  • A strong desire to learn and work in a team-based environment.
  • Strong verbal, organizational, and writing skills, attention to detail and ability to write clearly and complete collateral work in a timely manner. Fluency in another language (e.g., Spanish) is a plus.
  • Commitment to cultural humility when working with youth and families that are richly diverse in culture and background, sexual orientation, gender identity, race, religion, socioeconomic status, ability, etc.
  • 2 years (preferred) experience working with young adults with mental illness
  • Employee must be CPR and 1st Aid certified.
  • TB screen (negative results).
  • Valid driver’s license.
  • Reliable transportation
  • Basic Computer Skills
  • High School Diploma or GED required
