Outreach Case Manager
Shelter and Housing Division
Location: Pittsfield, MA
Schedule: Monday-Friday
Compensation: Starting salary $40,000/year
Position Summary
ServiceNet is seeking an experienced, forward-thinking Case Manager to join our Shelter and Housing Division. In this role, you will provide compassionate support and advocacy to individuals and families working to rebuild their lives. You’ll help guide our guests on their journey from homelessness to stability, connecting them to the community resources and services they need to achieve long-term success.
Key Responsibilities
- Conduct intakes and assessments upon guest entry into the program
- Support shelter and transitional housing participants by advocating for access to treatment, social services, medical and vocational support, housing resources, and other tools that promote self-sufficiency
- Meet weekly with guests (individuals or families) to review service plans, establish goals, coordinate referrals, and evaluate progress
- Collaborate with shelter staff and guests to help create and maintain a positive residential milieu
- Maintain timely, accurate documentation including case management notes, progress reports, and other required records
Qualifications
- Bilingual candidates (Haitian Creole / French / Spanish) encouraged to apply
- High school diploma or equivalent, plus two years of additional training or college coursework
- At least one year of experience supporting individuals living with low income, mental health conditions, and/or addiction
- Familiarity with local social service agencies and available community resources
- Valid driver’s license and acceptable motor vehicle record
