Position Summary:
Participate as a member of a multi-disciplinary team promoting a model of treatment informed by the principles of person-centered planning, trauma-informed and strengths-based service provision, and the belief in, and support for, resiliency, hope and recovery. Manage administrative responsibilities for the team.

Position Responsibilities:
- Work one-to-one with participants guided by the clinical staff and in accordance with the goals and objectives outlined in the treatment plan. Participate in annual and quarterly meetings. Assist participants in exploring their goals in preparation for treatment planning meetings. Provide meaningful interaction that promotes successful accomplishment of tasks and activities. Work with participants on skill development to achieve identified goals. Provide monitoring, as needed, to promote and maintain a safe living environment in the community for assigned individuals. Use strategies and interventions founded in evidence-based practices to restore and teach skills, as outlined in participants’ treatment plans.
- Provide MAP oversight for the team in accordance with MAP regulations. Keep MAP certification current.
- Provide Representative Payee services to participants on the team. Maintain updated Financial Assessments and Money Management Plans. Follow ServiceNet guidelines for managing budgets and social security funds, check requests, etc.
- Facilitate House Meetings in supported housing sites.
- Assist in administering team petty cash funds.
- Oversee team vehicle usage, need for service, mileage reports, etc.
- Review and follow individual Plans for Safety and Well-being, Crisis Plans, Risk Management Plans and any other relevant plans and protocols.
- Consult with clinical staff who are on-site and/or on-call regarding any crisis or risk situation.
- Complete all documentation within expected timeframes, including Service Notes, utilization reports, event reports and other documentation as required in accordance with established program standards. Assist in completion of monthly notes on participants in the electronic health record in accordance with Rehab Option guidelines.
- Other duties as assigned by Clinical Team Leader or Contract Director.
Position Qualifications:
- Relevant experience required.
- Bachelor’s degree or master’s degree preferred.
- Ability to work collaboratively with a multidisciplinary team.
- Vehicle, valid driver's license and MVR on file.
- CPR & First Aid and MAP certifications.
