Position Summary.
The Team Nurse for Adult Clinical Community Services Team provides medical, psychiatric, health and wellness consultation to the ACCS teams as well as direct services to select individuals in our outreach community and GLE programs. This position will be assigned a clinical caseload and will be responsible for writing and implementing psychiatric comprehensive assessments and treatment plans. Understanding of a respectful person-centered approach and care coordination within a multi-disciplinary team model is required. This position has a dual reporting relationship to both the Clinical Nurse Director and the Clinical Team Leader and functions as an integral part of both the management and nursing teams.

Position Responsibilities.
- Provide education, training and consultation to staff on health-related issues, medication administration and other medical/healthcare topics.
- Medical oversight, intervention and coordination of related services for individuals assessed to be in need of medical support through their formal treatment plan or updated team evaluation, both at residential sites and at other community settings
- Work one-to-one with identified individuals toward achieving health and wellness goals.
- Provide medication education, administration and oversight to medically complex individuals as needed with the ultimate goal of medication independence.
- Act as liaison to internal and community health providers and care coordination entities in order to facilitate treatment and ensure positive outcomes for individuals in all levels of care.
- Participate in meetings as assigned, which may include, but are not limited to: intakes, staff meetings, external treatment meetings, care coordination meetings and meetings with DMH and Crisis teams.
- Document all interactions, communications and assessments within required timeframes.
- Provide coverage for Clinical Nurse Director as needed in accordance with reasonable triage practice.
- Participate in nursing on-call rotation as assigned.
- Additional duties as assigned by the Clinical Nurse Director, Contract Director or Clinical Team Leader.

Qualifications.
- Current RN licensure in the Commonwealth of MA with a minimum of 3 years psychiatric nursing experience required.
- Demonstrated skills in professional clinical assessment and treatment plan writing and implementation.
- Experience in community based psychiatric and medical health treatment.
- Knowledge of, and commitment to, recovery based practices.
- Must meet LPHA qualifications.
- Valid driver’s license, vehicle and MVR on file.
