We're looking for an experienced Case Manager to help improve the overall health and well-being of people with persistent mental health and substance use disorders. Working in partnership with clinical and non-clinical staff on the Certified Community Behavioral Health Clinics team, you will see clients both in the community and at our Northampton and Greenfield offices.
As our Case Manager, you will:
- facilitate communication and coordination between your clients' mental health, substance use treatment, and medical care providers;
- help clients get the support services they need for successful recovery, including housing, employment, and legal assistance;
- build relationships with community providers and organizations to enhance clients' ability to get the care they need when they need it
You have been in Case Management for at least three years and enjoy the chance it gives you to make a significant difference in people's lives. As organized as you are compassionate, you know how to help people pull all the pieces together and become more self-sufficient. And when you spot barriers in their way, you don't stop until you've figured out how to get through.
You hold a Bachelor's degree in Social Work or a related field. And since your work will be taking you all over Franklin and Hampshire Counties, you also have a driver's license and use of a vehicle.
